Let’s break this down somewhat so I can explain some of the thinking of why I chose what I chose. To make it make sense, I’ll break them down all by usage. Cargo- carrying gear. Cargo is really where I cut down most of my weight without sacrificing effectiveness. I found a great bag that’s big enough to carry everything I need, with even some extra room left over. With my original bag, attachments to hold what wouldn’t fit, and internal bags to keep things organized, my cargo weight was well over 8 pounds! Essentially, cargo is all the stuff that holds your stuff that isn’t stuff you need to survive. I cut that down by half, saving me four pounds that I could devote to other gear and I actually have more room than I did before. I can now fit everything inside the bag – including the tent – and I still have some room. The tent adds 4. 8. I’m using an Osprey Atmos 6. Liters of space. I chose this particular bag because I already have an idea about how much space I need from using my Camelbak BFM for so many years. I originally got a 5. Rush 7. 2 backpack to replace it, after MANY people suggested it and read some really great reviews on it. After thinking it all through, it wasn’t going to work. In order to carry the amount of items I wanted, I’d have to add cargo space to the bag and it already weighed quite a bit. Plus, it’s a very military- looking bag, which I’d rather avoid if I could. It also seals up at the top and has a built- in cover for the rain. I also switched out all my internal bags for ultralight ones. For organization, I got a set of Eagle Creek packing cubes and an REI expandable packing cube. If I didn’t already have the REI bag, I would have gotten another set of the Eagle Creek cubes instead and separated things even more – and the whole set weighs about the same as the one REI bag does. Cutting tools. One of the most important things to have in a survival kit is a fixed- blade knife. I used to carry a SOG Seal Elite, and I even carried it in Iraq and Afghanistan, but for a bug out bag here at home, I wanted to see if I could cut down the weight and still have a very effective knife. After a LOT of research, I pretty much came back to the same thing but got the SOG Seal Pup instead. My original SOG Seal was 1. The pup weighs 5. When you consider the smaller sheath size, that saved me almost a quarter pound right there – and it’s a great little knife. Batoning a knife to build a shelter is a LOT of work so I wanted to get a really good hatchet. It came down to the Estwing E2. A Portsman’s Hatchet vs the Fiskars X7 1. Hatchet. I went back and forth with this one for a long time. I ended up with the Fiskars because it was over a quarter pound lighter (from what I could find on the web and assuming the sheath weight) and had even more 5- star reviews. I REALLY like the looks of the Estwing but cutting weight is a priority over aesthetics. Barely. As a backup knife, I have the Gerber Multitool that the Army gave me that I keep in one of the front belt pockets of the backpack and a small Boker Subcom F in my Survival- Tools bag. Because cutting medium- sized logs is a LOT of work with a hatchet and even more work with a knife and stick, I got a basic replacement chain saw, cut it so it was one length instead of a loop, and put a couple of strong key rings on the ends. I can use it for a very effective hand saw by either sticking a short branch in each end to make handles or tying a length of the 7. If you then tie some fishing line and some kind of weight, you could throw it up and over a branch up in a tree and cut it down without having to climb up. I keep the chain rolled up in a small Altoids- type of tin. Making fire. I think fire is one of the most important elements of survival so I have a lot of redundancy here. Slid into the back of my backpack is a sheet- sized Fresnel Lens. It weighs so little that it didn’t come up on my scale and since there’s a flat spot that it fits into nicely, it effectively takes up no weight. It’s a no- brainer to get one of these. Here’s a quick video I did showing how easy it is to start a fire with one of these if you have a sunny day and some dry stuff laying around: I have an old Brunton lighter that I threw in there because it’s not only a storm- proof lighter, it has an LED light built- in. No idea which model it is. I have two Doan Magnesium/Ferrocerium Firestarters. Don’t get the cheap stuff, get the Doan one. The Chinese knockoffs just don’t work as well and they’re about the same price. I keep one in my Survival- Tools Bag and one in the sharpening stone pocket of my SOG Seal Pup knife. I have a very small plastic container that has a couple of tea light candles, a couple of packs of matches, and some Tinder- Quik Fire Tabs that I’ve sealed inside short lengths of a large straw to keep them dry. I didn’t really need this but I threw it in anyway. In my Survival- Tools bag, I also have a small two- hole magnesium pencil sharpener. It’s great for shaving tinder from twigs and only weighs a quarter ounce. I’m not sure if the magnesium it’s made out of is pure enough to be used to help build a fire but it might. I carry a roll of toilet paper and a few small packs that came with some MREs I ate in Afghanistan. This stuff is great for starting fires. Also great if you happen to bring a girl camping. You’re like a rockstar if you bring toilet paper. Chicks dig toilet paper. Filtering/purifiying water. My go- to water filter system right now is the Sawyer Mini. It filters a LOT more water than pretty much everything out there, but to do that, you need to use the plunger it comes with. It can also be adapted to run inline with a Camelbak and it comes with a roll- up water bottle. I keep one Sawyer Mini in one of the front belt pouches and one in the Survival- Tools bag along with a plunger and a roll- up bottle. I have my cook set detailed below so I won’t go into it here but it can obviously be used to boil water. I have a Harley handkerchief that they gave me at the local shop on Veteran’s Day last year. It can be used to filter out all the big stuff and to soak up small puddles. You can also brush it on tall dew- covered grass and plants as you walk through and wring it out into your mouth, although that won’t taste too good. Much better than using your socks though. I have to be humble while someone lists my life accomplishments. Abel: You’re very well respected in this field. Intermittent fasting is such a hot and important topic that I think everyone should know about it. We wanted to make the book accessible to anyone, so it can be downloaded as a pdf, or read online like a book. We wanted it to be free, and people shared it like crazy through social media and other venues. For the book, I basically made myself a guinea pig for a year trying every type of intermittent fasting that seemed sane and reasonable. Then I recorded everything from blood work to photos to performance variable and cognitive measures. This approach was so radical because at the time what we were teaching was frequent eating. People thought I was crazy for doing the opposite of that. But it worked. Abel: Can you talk about what happened to you over the course of time trying intermittent fasting regimes? I am the ultimate test pilot of every new diet and training fad that comes out. I have a 7. 00 square foot gym in my house complete with testing equipment. It’s a mini exercise physiology lab. I test everything. When the online discussion of intermittent fasting were taking shape, there was a lot of strong opinions and inflexibility. I hadn’t seen anyone taking an objective non- religious point of view on it. So I decided to use intermittent fasting to help reinvigorate my athletic career. Some friends and I decided to run the 4. Penn Relays as master athletes. Normally, I was about 1. I needed to get down to about 1. Intermittent fasting was my solution. Can I lose a tremendous amount of body fat while preserving my lean muscle mass in a way that’s more sustainable? I started a daily journal recording every morsel of food and every workout along with all the tests and results. Then I published it. I’ve tried 6 or 7 different protocols from a once- a- week 2. Some protocols were fantastic, sustainable, and I could do them. Some of them were a total trainwreck, leaving me obsessing about food all day. I was fantasizing about Dairy Queen. But I ended up losing a tremendous amount of body fat. People say that’s impossible. I started in an 8% range and by the end my ultrasound was reading 3% body fat. Correct or not, you can see by pics that I was completely shredded. I reported on T levels and all the other things. It was a cool experiment for me and helped add a new tool to my nutrition tool box. THE PROS AND CONS OF INTERMITTENT FASTINGAs a trainer, there are times that I use intermittent fasting for certain purposes. But there are also certain people and situations that it’s not good for. There may be lifestyle stress, a new baby or a new job, and that’s not a good time to fast. If something works for you for now, that’s great. But to suggest it will work for everyone in every situation forever, I’m dubious of that. Abel: What sort of conclusions did you walk away with? People who are brand new to paying attention to nutrition shouldn’t start off with intermittent fasting. I’d actually start with more frequent eating, practicing making fantastic food choices and eating around your exercise appropriately. If you’re more advanced in your nutritional understanding, just make sure you start intermittent fasting when your lifestyle stressors are better managed. If your lifestyle stress is out of control and you’re training 6 times a day, you may not want to start intermittent fasting. You’re just setting yourself up for adrenal fatigue. In women there’s some caution as well. Women have a very delicate reproductive system that’s fine tuned to energy balance. If you cut down on calories too much and exercise hard, you end up upsetting the reproductive balance. It’s not just about reproducing, it’s about the feedback to the brain. It’s “crazy brain.” If it thinks you’re starving, your brain will produce any kind of thoughts that will get you to eat. You’ll end up obsessing over food and obsessing over body image. For me it’s not a part of my regular lifestyle because I have two young children at home. My wife doesn’t practice intermittent fasting and of course neither do they. So, I can’t be like, “Breakfast? You guys are on your own. Forget it, we’ll eat 6,0. Abel: On that subject of self- testing, it’s an exciting time for this because you can test all kinds of biomarkers for a relatively low price. What do you recommend for people to measure how well something is working for them? Coaching is essential if you’re pretty new to this. If you don’t know what to evaluate, it’s awesome to have someone to check in with. They have the experience and perspective that you don’t have about yourself. I don’t know why anyone would embark on a major lifestyle change without having someone to check in with. I’m not talking about taking out a second mortgage to hire a daily trainer. If you don’t know how to evaluate what’s working, find someone who does. Learn how to apply the scientific method. My kids use it every day. Will it work if I do this? We all have the scientific method in our heads. But the key is that we have to control some variables. If you want to see if intermittent fasting is working, it’s probably not the time to also change your training program. The biggest thing is attention and awareness. Really, it’s just paying attention to your life. If you’re trying intermittent fasting and you’re only paying attention to your abs, you’re missing the rest of the stuff. Like your mood. In the hard core training community (crossfit, powerlifting, bodybuilding, high intensity sports), it’s common to train ourselves to ignore physiological feedback. Your body is telling you to stop, but you’re training yourself to ignore that. That’s okay in a certain context, but in the rest of your life it’s important to listen to those cues. When someone says that if they listened to their hunger cues and ate until they were satiated, they’d be 4. There are ways of being able to cultivate this. Some people use meditation, but you don’t have to do that particular strategy. One thing we use is just purposely eating very slowly—Take 1. You see your attention open up, and it actually takes 2. Once you’ve mastered that, try eating to 8. That’s satisfied, whereas 1. The first thing is just find a way to get attuned to yourself so you can pay attention to all the things you need to pay attention to. And then get some coaching. Abel: When it comes to self- testing, a lot of people (including me) are too cavalier at first. Why is it important to have a coach or a guide through the process? It’s a difference between knowledge and wisdom. Knowledge is knowing stuff, wisdom is knowing what works. We’ve coached over 2. When you start coaching that many people from all walks of life, you start seeing some amazing patterns emerge. You start seeing these clients who are accepting info from all over the place—online, media, friends, etc. They need help curating the information that’s out there. When people are cavalier in the beginning, it’s because they don’t know where to start. It’s because they don’t have the wisdom. Not to find out what’s new, what’s next, what’s out there, but to figure out what works. Other treatments include a low- fat diet to prevent attacks and vitamin K for itching, jaundice, and bleeding tendendes due to vitamin K deficiency. Treatment during an acute attack may include insertion of a nasogastric tube and an I. V. line and, possibly, antibiotic and analgesic administration. A non surgical treatment for choledocholithiasis involves placement of a catheter through the percutaneous transhepatic cholangiographic route. Guided by fluoroscopy, the catheter is directed toward the stone. A basket is threaded through the catheter,opened, twirled to entrap the stone, closed, and withdrawn. This procedure can be performed endoscopically. Ursodiol (Actigall), which dissolves radiolucent stones, provides an alternative for patients who are poor surgical risks or who refuse surgery. Special considerations or prevention Patient care for gallbladder and biliary tract diseases focuses on supportive care and close postoperative observation: Before surgery, teach the patient to deep- breathe, cough, expectorate, and perform leg exercises that are necessary after surgery. Also teach splinting, repositioning, and ambulation techniques. Explain the procedures that will be performed before, during, and after surgery to help ease the patient's anxiety and help ensure cooperation. After surgery, monitor vital signs for signs of bleeding, infection, or at electasis. Evaluate the indsion site for bleeding. Serosanguineous drainage is common during the first 2. If, after a choledochostomy, a T- tube drain is placed in the duct and attached to a drainage bag, make sure that the drainage tube has no kinks. Also check that the connecting tubing from the T tube is well secured to the patient to prevent dislodgment. Measure and record T- tube drainage daily. Monitor intake and output.
